Quick tips on how to get ready for your visit with Texas Cardiovascular.
Preparation for your appointment:
If you are a new patient to Texas Cardiovascular, please download and print the patient forms. You will find the link to download patient forms under Quick Links. You may bring your completed forms to the office at the time of your appointment.
If you are a returning patient to Texas Cardiovascular, we require updated patient information at least once a year. You may download the patient information sheet and bring the completed form to your appointment.
In order to ensure timeliness of your appointment, please arrive at least 15 minutes prior to your appointment time.
Please bring the following items to your appointment:
- Current insurance card(s)
- All of your current medications (in their original containers)
- Current driver’s license or picture ID
Medical Records:
In order for us to provide you with the highest level of care, you must provide us with all necessary medical records from your referring physician, primary-care physician, and/or hospital stay.
Payment Options:
- We collect co-payments, co-insurance, and deductibles at the time of service. Please be prepared to make payment in full at the time of service unless arrangements with your physician’s office manager have been made prior to your visit.
- If you have a question regarding your benefits for a medical specialist, please contact your insurance company.
- For an estimated quote of services, please call us at (512) 617-6000 and ask for the office manager; indicate the physician you are scheduled to see and the location.
- We accept cash, check, and all major credit cards.
Preparation for your diagnostic test or procedure:
Many tests and procedures require fasting or medication in order to perform the study.
If you have not received instructions prior to your diagnostic test or procedure, contact the nurse of your physician prior to your appointment.
